Abstract

There are some clinical situations wherein delay in provision of blood through routine processing unduly jeopardize the patient. In these circumstances, it may be necessary to issue incompletely processed blood. To use such blood, the criteria of urgent transfusion must be made and the risk of transfusion reaction must be evaluated.
On the basis of the frequencies of Korean blood group systems including red cell antigens and irregular antibodies, we evaluate the probability of safe blood transfusion when pretransfusion tests are abbreviated for patients who have to recieve transfusion urgently. And then we conclude as followings:
1. When radom blood is used without any pretransfusion testing, 54.67% of these trasfusions would be compatible. When ABO groups are matched, 99.95% would be compatible. Matching of Rh improves the odds another 0.01%, while cross-matching increases the safety only 0.005%.
2. We think that blood must be issued only after completion of ABO and Rh typing if tie patient requires immediate transfusion.
